RN On Call / Job Share

Must be RN with Homecare and Hospice experience, deemed IV competent. Monday through Friday 4 pm to 8 am.  8 pm to 8 am Saturday and Sunday (including holidays). Each team member is scheduled 3 or 4 days every other week. Triage calls from home and emergent home visits. 

$10/hour for on call plus competitive visit rates and mileage. 

Home Health Aide Supervisor

RN with a minimum of 2 years homecare experience to supervise the Home Health Aide program.  Responsibilities include recruiting and orienting new HHA's, planning in-service education, supervising and evaluating HHA's competency and quality of care during home visits, and carrying a HHA-only patient caseload.  Additionally, the HHA Supervisor will participate in the agency's Wellness Programs.

Successful candidate will have a thorough knowledge of CT Home Health licensing Regulations and excellent leadership / communication skills.  Previous experience as HHA Supervisor preferred.                           

Primary Care RN

Case manage home bound patients for our "Care of the Sick" program.  Full time position. Must be RN with unrestricted license to practice in CT, and have at least 1 year's experience in an acute or sub-acute setting.  Seeking candidates with homecare experience.  Duties include patient assessments, and managing the plan of care.

Hospice / Palliative Care RN

Seeking experienced RN's with Hospice and Homecare experience  who are committed to the Hospice philosophy, and know what a difference they can make to the lives of patients and their families. 

Full time exempt position.

Must have an unrestricted license to practice in CT, and at least 1 year's experience in an acute or sub-acute setting.
