The Hospice Program’s
mission is founded on the belief that the alleviation of
symptoms related to
the final stages of terminal illness
will allow patients and families an
opportunity to enjoy
quality of life and to prepare for death in ways that
are physically, emotionally and spiritually
satisfactory for them.

Based on their assessment of a patient’s needs, our
clinical hospice team develops an individualized plan of
care to manage the patient’s symptoms and add to his or
her comfort in a way that fosters quality of life
through the last moments of life.

emotional, psycho-social and financial stresses

Pastoral Care Coordinator - provides spiritual
support and facilitates community clergy

Registered Dieticians - consult to enhance
nourishment and nutritional needs.

When a hospice
patient dies, members of the hospice team offer personal
and group support services. Bereavement services
are provided for families and loved ones who have
experienced a loss and to the community at large. |
