services are provided to patients of all ages suffering
from a range of health issues. Under the care of a
physician, patients may receive home care upon release
from the hospital, nursing care facility or other care
facility. A Nurse Intake Coordinator
processes all referrals and nursing services are
available 24 hours a day, 7 days a week by calling
