Based on the Hospice
Team's assessment of a patient's needs, our clinical hospice team
develops an individualized plan of care to manage the patient's symptoms
and add to his or her comfort in a way that fosters quality of life
through the last moments of life.
A Hospice Primary Care
Nurse is assigned to each patient and family, and coordinates the
comprehensive care of the patient and family. Our nurses are
specially trained in managing the symptoms that accompany a serious
illness and are experts at managing pain.

Director assures the program's philosophy and goals are
met.
Medical Director
assures quality of care.
Skilled Nurses
coordinate comprehensive care through collaboration with the
other team members specializing in symptom management at end
of life care including I.V. medication management in the
home.
Medical Social Workers
assist with the emotional, psycho-social and financial
stresses that may accompany a life-limiting illness.
Physical, Occupational
and Speech Therapists maximize safety and comfort when
needed.
Spiritual/Pastoral Care
Coordinator provides spiritual support and facilitates
community clergy resources.
Registered Dietitians
consult to enhance nourishment and nutritional needs.
Home Health Aides
assist with personal care and homemaking.
Volunteers provide
companionship for patients and respite for loved ones.
