The
Hospice Program's mission is founded on the belief that the alleviation
of symptoms related to the final stages of terminal illness will allow
patients and families an opportunity to enjoy quality of life and to
prepare for death in ways that are physically, emotionally and
spiritually satisfactory for them. An interdisciplinary team assists the
patient and family in achieving these goals and provide bereavement care
to survivors in order to facilitate a healthy grieving process.
Our Hospice Program provides a full range of
services tailored to meet the needs of each patient.
We accept
referrals from patient's physicians, and other health care
professionals. Although many hospice care patients live in a personal
residence, some patients live in nursing homes, assisted living
residences. and retirement communities. Our hospice care is available at
all these locations.
Our Goals:
To provide physical,
emotional social and spiritual care for dying
persons/families including pain management and relief of
symptoms, 24 hours per day, 7 days per week via an
interdisciplinary team of professionals and volunteers
To integrate Hospice
care principals into services provided by the health
care delivery system.
To provide educational
service and consultation in the areas of Hospice care,
death and grief.
To provide supportive
services and counseling for bereaved individuals,
families and groups.
