 Home Health services are
provided to patients of all ages suffering from a range of health
issues. We work with patients, families, and physicians to
outline an individualized plan of treatment specific to a patient's
needs.
Our
nurses safely provide many skilled services to patients in their homes.
Under the care of a physician, patients may receive home care upon
release from the hospital, a nursing care facility, or other care
facility. Our Home Care team is more highly trained than ever
before. One of our roles is to instruct the patients how to take
better care of themselves in order that they have a better quality of
life by staying home. By VNA going into the home and making it safer,
and getting the patients involved in their own health care regimen,
patients are staying healthier longer and able to remain in the
