The VNA Health at
Home Hospice Team provides care for patients with serious chronic or
life limiting illnesses. We focus on the whole person, meeting medical,
emotional and spiritual needs of patients and families during this
difficult time. A team of experts work with the patient, family and
medical team to develop a care plan that meets the needs of each
individual patient as it relates to pain management and symptom control.

When to
make a referral
As soon as the patient is ready to no longer seek
active treatment for his/her illness a referral should be made
to the hospice program. Early referrals ensure that the patient
and family will benefit fully from the hospice program services
and experience comfort and dignity during their final days,
weeks or months.
How to make
a referral
Anyone can make a referral to the VNA Health at
Home Hospice Program – a patient, family member, medical staff
member, or friend. Patients will be evaluated to determine if
the hospice benefit is appropriate. An order from a physician
is required to admit the patient into the hospice program.
